You determine whether a creature has changed shape, either from Change Shape through a polymorph spell or effect. If the creature can change its form, but is in its true form, you are aware of this, but you can’t determine the shapes it can take. When you Sustain the Spell, you can observe a different creature, or study the same creature to see its true form and learn whether its changed shape is through an ability or spell.
